See below for ERSRI Model QDROs that correspond to the benefit option you selected at retirement (or plan to select when you retire) and your employment type.
Note: There are certain provisions which must be included in the QDRO. In addition, there is certain language which should not be contained in the QDRO that the Plan, as a defined benefit public pension plan, would not be able to accommodate. QDRO Model - Maximum Benefit
The QDRO models listed below are for retirees who selected the maximum benefit form of payment.
QDRO Model Max State and Teachers
Use this model if you are a retired state employee or retired teacher who selected the maximum benefit form of payment, or if you are an active state employee or teacher and your ex-spouse’s benefit is to be calculated as if you selected the maximum benefit form of payment.
QDRO Model Max Municipal
Use this model if you are a retired municipal employee who selected the maximum benefit form of payment, or if you are an active municipal employee and your ex-spouse’s benefit is to be calculated as if you selected the maximum benefit form of payment.
QDRO Model Max Police and Fire
Use this model if you are a retired public safety plan member who selected the maximum benefit form of payment, or if you are an active public safety plan member and your ex-spouse’s benefit is to be calculated as if you selected the maximum benefit form of payment.
QDRO Model - Option 1
The QDRO models listed below are for retirees who selected Option 1 form of payment.
QDRO Model Option 1 State and Teachers
Use this model if you are a retired state employee or retired teacher who selected the Option 1 form of payment, or if you are an active state employee or teacher and at the time of your retirement you will be required to elect the Joint and Survivor Option 1 form of payment and name your ex-spouse as beneficiary.
QDRO Model Option 1 Municipal
Use this model if you are a retired municipal employee who selected the Option 1 form of payment, or if you are an active municipal employee and at the time of your retirement you will be required to elect the Joint and Survivor Option 1 form of payment and name your ex-spouse as beneficiary.
QDRO Model Option 1 Police and Fire
Use this model if you are a retired public safety plan member who selected the Option 1 form of payment, or if you are an active public safety plan member and at the time of your retirement you will be required to elect the Joint and Survivor Option 1 form of payment and name your ex-spouse as beneficiary.
QDRO Model - Option 2
The QDRO models listed below are for retirees who selected Option 2 form of payment.
QDRO Model Option 2 State and Teachers
Use this model if you are a retired state employee or retired teacher who selected the Option 2 form of payment, or if you are an active state employee or teacher and at the time of your retirement you will be required to elect the Joint and Survivor Option 2 form of payment and name your ex-spouse as beneficiary.
QDRO Model Option 2 Municipal
Use this model if you are a retired municipal employee who selected the Option 2 form of payment, or if you are an active municipal employee and at the time of your retirement you will be required to elect the Joint and Survivor Option 2 form of payment and name your ex-spouse as beneficiary.
QDRO Model Option 2 Police and Fire
Use this model if you are a retired public safety plan member who selected the Option 2 form of payment, or if you are an active public safety plan member and at the time of your retirement you will be required to elect the Joint and Survivor Option 2 form of payment and name your ex-spouse as beneficiary.
QDRO Model Multiple Beneficiaries
The QDRO Model for multiple beneficiaries is for active state employees or teacher members only.
QDRO Model Multiple Beneficiaries
Use this model if you are an active state employee or teacher member and will be naming your ex-spouse and another beneficiary as equal benefit recipients under Option 1 or 2 either before or after retirement. Please contact our office regarding the election of Multiple Beneficiaries as there are restrictions associated with this election.
